Submit your application form and enclose:
- Official transcript of your mid-term results
(or a photocopy bearing the original signature, school stamp or seal)
- Bring together two signed letters of reference:
a) From your hockey team coach, which includes a description
of your leadership on the team, sportsmanship and dedication
to hockey
b) From a teacher, school counsellor or principal certifying
your academic achievement, positive attitude and school citizenship
- One page (maximum 250 words) essay on one of the
following topics:
a) The Impact of Hockey on Canadian Society
b) Greatest Hockey Memory
c) The Future of Women's Hockey in Canada
d) How does volunteering benefit your community?
